I grant Kids by the Hour the right to take photographs of my child.
Section 65C-22.006(2), F.A.C., requires a current Physical examination (form 3040) and immunization Record (form 580 or 681) within 30 days of enrollment
Section 402.325(5),F.S., requires that parents receive a copy of the Child Care Facility Brochure, “Know Your Child Care Facility”
Section 65C-20.010(6)(c)2., F.A.C. requires that parents are notified in writing of the disciplinary practices used by the childcare Facility.

ONE TIME USER policy: Child may come for ONE visit with no registration fee for that visit.

On 2nd visit you will owe $100 one-time fee per child.

Step Two: Provide Photo ID

Bring a valid photo ID.
We’ll make a quick copy of the enrolling parent or guardian’s driver’s license or photo ID for our records.

Step Three: Pay the Enrollment Fee

Enrollment Fee: $100 per child (one-time)
This fee supports administrative processing and enrollment setup.

Enrollment fees are sometimes reduced or waived. Contact us for details.

Additional Forms — No Rush

You have 30 days to submit these forms after enrollment.

There’s no pressure. These documents can be turned in during your first month.

Required Health Forms:

Form 3040 (Physical Examination – yellow)
Form 681 (Immunization Record – blue)

Both medical forms are available through your child’s pediatrician.

Important Fees at a Glance

Registration Fee

$50 per child, due every February
Supports art supplies, books, learning tools, and classroom materials

Registration and supply fees are non-refundable.

Our Hours & Flexible Care

Care for children ages 1–12

Monday–Thursday: 7:00 AM – 10:00 PM
Friday–Saturday: 7:00 AM – 12:00 AM

Drop-in care, extended hours, and flexible scheduling are available to support real family needs.
